Can someone explain to me how the miles figure is computed? If it's based on fuel consumption and fuel efficiency of the whole transportation fleet, that requires assumptions about what parts of the fleet are doing how much of the driving under various economic conditions, doesn't it? And where do they come from?

Roughly speaking, the data comes from traffic monitoring surveys by the states (actual roadway measurements). They have continuous measurements at a network of high traffic points and they do occasional measurements at a much large number of points, and then they extrapolate. The results seem to be pretty decent.
